In-Home Support Services

Professional Nurses - Registered Nurses, licensed by the Commonwealth of Pennsylvania, work with our clients every step of the way providing:

Assess and Manage Cases to determine the level of care needed

Supervise all direct care workers           Give Injections

Instruct and Educate about the disease process, i.e. diabetes, 

hypertension, etc. 

Nutritional Counseling / Emergency Care Visits / Social Service Referrals

24/7 On Call for medical questions.

Home Health Aides - Paraprofessionals trained in a Medicare approved course of 80 credit hours and certified by Community Care Center.  An additional 26 hours training  is available for the care of the developmentally disabled.   These direct care workers can provide:

Bath Service       Personal Care       Meal Planning and Preparation

Assistance with Treatment Prescribed by the Care Plan      

Assistance with Activities of Daily Living

Homemakers/Companions - These trained professional can:                                                 

                               Assist with Meal Preparation        Provide Companionship                        

                             Perform light housekeeping, do laundry, go food shopping

Family Respite Care  - Qualified caregivers stay with loved ones for weekends, holidays and vacations providing a break from care giving duties.
